To treat a cavity your dentist will remove the decayed portion of the tooth and then “fill” the area where the decayed material was removed.

Fillings are also used to repair cracked or broken teeth and teeth that have been worn down from misuse (such as from nail-biting or tooth grinding).

What Steps Are Involved in Filling a Tooth?

First, the dentist will inject a local anesthetic to numb the area around the tooth. Next, the dentist will use a drill, air abrasion instrument, or laser to remove the decayed area. The dentist’s comfort level, training, and investment in equipment — as well as the location and extent of the decay — will determine the choice of instrument.

Then, the dentist will probe and test the area to confirm that he or she has removed all the decay. Once the dentist clears the decay, he or she will clean the cavity of bacteria and debris to prepare the space for the filling. If the decay reaches near the root, the dentist will first place a liner made of glass ionomer, composite resin, or another material to protect the nerve. Finally, after placing the filling, the dentist will finish and polish it.

The Tooth-Colored Filling Procedure

Tooth-colored fillings require several additional steps. First, your dentist removes the decay and cleans the area, then applies the tooth-colored material in layers. Next, your dentist uses a special light to “cure” or harden each layer. Once your dentist completes the multilayering process, they shape the composite material to the desired result, trim off any excess material, and polish the final restoration.

What Types of Filling Materials Are Available?

Today, several dental filling materials are available. Teeth can be filled with gold; porcelain; silver amalgam (which consists of mercury mixed with silver, tin, zinc, and copper); or tooth-colored, plastic, and glass materials called composite resin fillings. The location and extent of the decay, cost of filling material, patients’ insurance coverage, and your dentist’s recommendation assist in determining the type of filling best for you.

Do dental fillings hurt?

Most fillings are quick and comfortable. Your dentist will numb the area, so you may feel slight pressure but not pain. Mild sensitivity to hot or cold is normal for a few days after the procedure.

How long do dental fillings last?

The lifespan depends on the material used and your oral habits.

  • Composite (tooth-colored): 5–10 years

  • Amalgam (silver): 10–15+ years
    With good brushing, flossing, and regular checkups, fillings can last even longer.

What are the signs I might need a filling?

Common signs include:

  • Tooth sensitivity to hot, cold, or sweets

  • Visible dark spots or holes

  • Pain when biting

  • Food getting stuck in one area
